Form 1095-B Mailed Out

Form 1095-B was mailed out on Monday, February 6 to non-Medicare Fund Participants enrolled in the Indemnity PPO Medical Plan in 2022. The form is used to report certain information to the IRS and to taxpayers about individuals who are covered by minimum essential coverage and therefore are not liable for the individual shared responsibility payment.

If you need a corrected form and were enrolled in the Indemnity PPO Medical Plan or do not receive the form in the next two weeks, you can request a copy by email.
