With tax season upon us, we anticipate many members are wondering how to get a copy of their W-2. Below is a list of some of our bargaining units and the steps to take to get a copy of your W-2.
ALBERTSONS/VONS/PAVILIONS
Go to www.mytaxform.com and enter Employer Code 11202. If this is your first time logging in your PIN will be your Employee ID.
CVS PHARMACY
Go to Colleague Zone and click on “Access your 2023 W-2 Online” and then click on “Access Your 2023 W-2 FormW-2 Now”.
GELSON’S
Contact your store director and they will contact payroll who can reissue your W-2 immediately.
KAISER PERMANENTE
KP employees can print their W-2s from the HRconnect website. If the employee no longer works for KP and/or have moved and didn’t receive their W-2, they can call the National HR Service Center at 1-877-457-4772 to request a duplicate. The website is https://hrconnect.kp.org/wps/myportal/hr . Click on the “Money” tab. Click on Taxes and W-2 link.

Log into MyInfo
• In the “Me” Section, select the “My Tax Form” tile to access your tax forms
• Click the link to mytaxform.com
• From the MyTaxForm home page you can:
» Elect to receive your W-2 online
» From the Menu, select My Account
» Verify Email address(es), Address and Phone Number
» If desired, you can select Online Delivery or you can remove online delivery
at any time.
» Retrieve your current or past W-2 by selecting the checkbox for the W-2 you wish to download, view, postal mail or fax.
RITE AID
W-2 Forms are online through Workday. If you have questions, email the Payroll Tax Department at taxw2rq@riteaid.com.
