Notify your union of major life changes
Lots of things need to happen when you experience a major life change such as moving, getting married, having a baby or getting a divorce.
High on the list should be notifying your union and trust fund within 30 days of the event so your benefits can be adjusted accordingly.
If you move, you might have told your friends, your employer and your family, but have you told your union? If not, the union can’t contact you to distribute benefits or keep you informed about key issues.
If you get married or have a baby — either by birth or adoption — your new family members deserve to share the outstanding benefits you receive as a UFCW member. Request an Add/Change form from the union. Return that form with a copy of the marriage certificate, birth certificate or adoption papers to declare a new dependent.
The same should be done if you get a divorce. If you fail to report this information, your ex-spouse will continue to receive coverage and could make claims on your benefits. If they are not eligible, you could be found responsible for reimbursement of claims paid. Submit an Add/Change form with a copy of the dissolution judgment to show exactly when the marriage ended.

Notify your union within 30 days of any of the following events in your life:
* Marriage
* Baby
* Adoption
* Change of Address
* Divorce
* Change of Guardianship
* Death in the family
